We provide practical, evidence-based support throughout the NHS Continuing Healthcare (CHC) process. Every situation is different, so we tailor our input to the level of support you need.

Step 1

Eligibility advice

We review your situation against the CHC criteria and offer honest, realistic feedback about whether CHC is likely to be appropriate.

  • Initial review of health and care needs
  • Clear explanation of CHC criteria
  • Advice on whether to request a Checklist
Step 2

Documentation support

We help you gather and present clear evidence from care records, risk assessments and clinical reports so that the full picture of need is visible.

  • Review of care home / community records
  • Support to evidence day-to-day needs
  • Guidance on useful risk documentation
Step 3

Assessment preparation / representation

We prepare you for Checklist and DST meetings and, where agreed, can also represent you during the process. This includes guidance on the types of questions you may be asked and how to describe needs clearly and consistently.

  • Preparation for MDT / DST meetings
  • Support to describe needs in each domain
  • Practical tips and optional representation in meetings with professionals
Step 4

Appeals support

If you disagree with a CHC decision, we can review the outcome, advise whether an appeal is realistic and support you with written submissions.

  • Review of decision letters and evidence
  • Advice on strength of potential appeal
  • Support with written challenge or appeal routes
