Our role

Pathway CHC Support Limited exists to help individuals, families and organisations make sense of NHS Continuing Healthcare (CHC). We provide independent guidance, realistic expectations and clear support through assessments and appeals.

We understand that CHC can feel complex and overwhelming. Our aim is to break the process down into manageable steps and to help you present clear evidence about day-to-day health needs.

Our approach

• Independent: we are not part of the NHS and offer impartial support.
• Evidence-led: we work in line with the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care.
• Person-centred: we focus on the individual, their needs and what matters to them and their family.

Chief Executive Statement

With more than a decade of experience supporting vulnerable adults, we bring specialist expertise in Continuing Healthcare (CHC) assessments across a wide range of healthcare environments. Through our work alongside Integrated Care Boards (ICBs), we observed persistent confusion surrounding CHC funding eligibility. National research also highlights significant regional inconsistencies in assessment outcomes, reinforcing the need for clearer, more equitable support.

These challenges motivated us to establish Pathway CHC Support Limited. Our team is dedicated to simplifying the CHC process, helping individuals and families understand what the criteria mean in real terms and how best to evidence day-to-day needs.

We draw on strong clinical insight, a thorough understanding of complex medical conditions, and real-world experience of how they affect daily living. This combination of clinical expertise and practical knowledge strengthens the support we provide and ensures that our clients receive clear, informed and realistic guidance at every stage.
